Football Sports Physicals need to be turned in when you register. You can go to any Doctors Care and tell them you are with SGS - your sports physical will be 30.00. If you are registering and have everything turned in but the physical please get in touch with Jennifer 243-5466 so that she can schedule you a time for a physical at Allstate.

**Your registration will not be complete/childs name will not go on a roster until you have COMPLETED your registration! **

Register now until July 10th  -10am-2pm at Allstate- Moonville office. See Jennifer Martin

Late Registration will be Saturday July 10th 10am-2pm at Allstate- Moonville where a $25 late fee will be added to your registration cost. No Exceptions!

Registration is for Tackle Football ages 6-12, Cheerleading ages 5-12, Fall Baseball and Softball league age 5-12. Prices will vary and will be posted soon.   

Returning Baseball/Softball $40 New BB/SB $65    Football $85  Cheer $100
